Heritage Health - Streator was started in 1964 and later expanded to 130-bed facility, providing not only skilled long-term care but now including "Restore to Home" Therapies and Nursing services to bridge your needs between hospitalization to home. We now average approximately 124 discharges back into the community, either home independently or home with services.
"Families trust us because of our reputation," says Administrator Janette Strabala. "We have a friendly, kind, and caring staff, and our facilities are always clean and pleasant. We try to be a home-away-from-home for our residents."
Our facility boasts many advantages, including a beautiful memorial garden for our residents to enjoy. Our residents also can trust in the longevity of our staff and the continuity in care that comes from high staff loyalty. In addition, we staff a Skilled Nursing Service in-house, 24 hours a day, 7 days a week.
Residents also enjoy many activities, 7 days a week, both within our facility and within our community. This is just one more way we make our residents feel at home.
We also provide wireless internet service for guest and visitor use.
